Full Guide How to make Unique Disability ID Card (UDID) for THALASSEMIA PATIENTS!

GOOD NEWS FOR THALASSEMIA PATIENTS

Thalassemia is now included in Department of Empowerment of Persons with Disabilities, Ministry of Social Justice and Empowerment, Government of India.

That means Person suffering from Thalassemia will get Unique Disability ID Card (UDID).
He/She will also get reservation in handicapped quota in Jobs as well as applicable for various Schemes for Persons with Disabilities and Scholarship schemes.

There are two ways you can Apply for this:

  1. Online 
  2. Offline 

Online Method

Before Starting for applying keep your Scanned copy of passport size photograph, Scanned copy of your signature, Scanned copy of your address proof and ID proof (Aadhar Card, Driving License, Passport, Voter Id Card, Ration Card and Pan Card) and Scanned copy of your Caste Certificate (if you have one) ready.

Step 1:-

Click on this link and Click on Apply for Diability Certificate & UDID Card.



Step 2 :-

Fill your Personal details Correctly and Upload your Photo, Signature and Address Proof.

Note: 
  • Scanned copy of Photo should be in jpeg, jpg, gif or png format of size 15KB to 500KB.
  • Scanned copy of Signature should be in jpeg, jpg, gif or png format of size 3KB to 500KB.

Step 3 :-
  • Click Next
  • Select "no" for Do You Have Disability Certificate?
  • Select Disablity type to "Thalassemia".
  • Select Disablity by Birth or Since (year) according to history.
  • Enter the Name of Hospital where you are undergoing treatment.
  • Select "Herediatry" in Disability due to.
  • Click NEXT
 
Step 4 :-
Enter Your Employment Details according to you than Click NEXT.

Step 5 :-
  • Enter your Indentity details correctly.
  • Select Indentity proof from option below and enter your aadhar number correctly.
  • Enter the code from image given.
  • Select " I have read and agree to the Terms and Conditions."
  • Click Proceed.

After Submitting you will get a Acknowledgment of Submission keep your Enrolment number and Receipt and you can track your application status on the same website.


Offline Method



Step 1 :-

Click on this link and Click on Apply for Diability Certificate & UDID Card.

Click on Download Form and Take Print Out of the same.







Step 3 :- 
Fill your from Correctly, attach everything and Submit to CMO Office/Medical Authority Near you.
